Portable Infusion Pumps Let More Children Finish IV Antibiotics at Home

Tamperelainen reports that the children's home hospital has started using antibiotic infusion pumps so more children who need intravenous antibiotics can finish treatment at home. The pumps deliver a steady, 24-hour infusion, so a nurse needs to visit the family only once a day to change the antibiotic solution instead of several visits. Maria Pohjanpää, the home hospital's responsible physician and pediatric specialist, says children must be in generally good condition and clearly responding to treatment before care continues at home. The service currently uses four antibiotics: vancomycin, piperacillin-tazobactam, benzylpenicillin and cloxacillin. Families have adapted well to managing the device in everyday life, getting outside, attending school or doing hobbies while carrying the pump in a backpack.

The rollout includes two delivery methods. Patient-specific pumps are prepared in the hospital pharmacy's sterile cleanroom, while ready-to-use devices with prefilled drug cassettes allow faster starts outside pharmacy hours. Pharmacist Henna Väänänen says same-day pharmacy orders are usually possible. Heli Moilanen says aseptic preparation in the cleanroom extends shelf life, so pumps for several days can be supplied in advance. The pump programme is part of a study run through the Children's Medical Research Center PeeTU, which is monitoring safety, practical challenges, cost-effectiveness and impacts on family quality of life and parents' work absence. Clinicians say it also frees up hospital beds and lowers costs by reducing inpatient days.
